ANGST FOR Å FØDE: ÅRSAKER OG KONSEKVENSER - en oppfølgingsstudie av gravide og fødende på Akershus universitetssykehus it t Malin Eberhard-Gran, lege og senior forsker
BAKGRUNN: Inntil 20 % av alle gravide har sterk uro eller frykt for å føde Alvorlig angst for å føde: ~6-10% Tokophobia: ~ 2 % Mange ønsker keisersnitt itt på grunn av uro eller frykt for å føde Keisersnitt større risiko for den fødende og opptar store ressurser på sykehusene
Angst under svangerskapet har vært relatert til: - Langvarig fødsel - Økt behov for smertestillende - Økt risiko for akutt keisersnitt ese s - Postpartum depression - Påvirkning av tilknytningen mellom mor og barn
TIDLIGERE STUDIER: Metodologiske svakheter: små utvalg dårlig svarprosent ikke-validerte psykometriske måleinstrumenter ingen oppfølging g
HOVEDHENSIKT: Å studere 1) risikofaktorer for angst knyttet til fødsel og, 2) hvordan angst påvirker forløsningen og barnet.
Risikofaktorer: Angst / depresjon Tidligere traumatiske fødsler Tidligere seksuelle overgrep Sosiodemografiske faktorer Somatisk helsetilstand Fødselsangst Konsekvenser: Varighet av forløsningen Forløsningsmetode Risiko for fødselsskade Blødningsmengde /varighet Barnets fødselsvekt
Andre temaer som det er mulig å studere: posttraumatisk stress etter fødselen langtidseffekter av fødselstraumer sammenhengen mellom smerte og angst I forbindelse med fødsel faktorer relatert til legemiddelbruk (inkludert naturmidler) under svangerskap sammenhengen mellom selvrapportert søvn og depresjon under svangerskap og barseltid hvordan ulike typer av hodepine påvirkes av svangerskap og omvendt alkoholbruk under svangerskapet fysisk aktivitet under svangerskap og forløsningsutfall
MÅLPOPULASJON: Alle kvinner som fødte på Ahus i perioden mars 2009 til og med februar 2011 (N= 6244)

MENTAL HEALTH MEASURES: the Wijma Delivery Expectancy/ Experience Questionnaire a fear of childbirth is also measured using a numeric rating scale the Edinburgh Postnatal Depression Scale the Hopkins Symptom Check List the Anxiety Disorder Questionnaire Kendlers Life Time Depression Scale the Prenatal Attachment Scale the Mini-IPIP scale the Bergen Insomnia Scale various standardized questions about expectations and feelings associated with childbirth various pain measurements
reproductive factors OTHER STUDY FACTORS: circumstances during the delivery (mode of delivery, anaesthesia, offspring vital status etc.) negative life events socio-demographic variables information concerning diseases symptoms of pelvic girdle pain use of medication, alcohol, cigarettes, narcotics body mass index (BMI) physical activity social networks relationships with husbands/significant others/partners mistreatment or abuse (the Abuse Assessment Screen)
