FENTANYL: 1) ACB, kv, 41 år, 56 kg: fentanyl 5,0 mg + kurare (Anesthesiology 1981:54:341-3) 2) ACB, kv, 45 år, 66 kg: fentanyl 4,8 mg + kurare ( Anesth Analg 1980:59:948-9)
- 56 curariserte pasienter (m/ cuff på arm,dvs uten curare) - BIS mellom 60 70 m/propofol TCI i 20 min, ingen kirurgi - Kommando hvert min: Klem hånden min to ganger - Kerssens C et al. Anesthesiology 2003:99:570-5 37 pasienter (66%) utførte kommandoen Hadde sign høyere BIS (67 vs 63) og sign høyere BT (MAP;86 vs 83) 9 pasienter (13%) husket dette etterpå ( recall ) 8 hadde utført kommando, 1 hørte navn ingen kommando utført 5 var OK, 4 hadde lett ubehag (vanskelig å svare, usikker på forløp videre, beroliget av tiltale) Jmf. m/ ingen recall pas: høyere hjertefrekvens (73 vs. 67) flere utførte kommandoer ( 30% av alle kommandoer, versus 10%) samme BIS verdier

Hvordan monitorere awareness? - Per-operativ våkenhet kan ikke foutsies! - Ingen monitor kan gjette hva en kirurg vil finne på! Men. - Dyp søvn minker sjansen for at plutselig stimulering skal resultere i våkenhet. - Rask erkjennelse av våkenhet kan hindre awareness

Konklusjoner: BIS kan brukes til å monitorere søvn og minke sjansen for awareness Unngå BIS > 60 for lengre perioeder (> 4 min) Usikkert om vi samtidig kan spare medikamenter (BIS mellom 50 og 60) og redusere BIS faren (BIS mellom 40-50) For dyp søvn (BIS < 40) er muligens uheldig
