1 Date: February 27, 2013 Acme Electric Customers Product Advisory Notice Transformer Mounting Brackets Acme Electric has been notified of a potential quality issue with the mounting bracket on Acme single phase general purpose and buck boost transformers in sizes ranging from 0.25 kva to 2 kva. Please refer to the attached letter for specific details on this recall, how to identify the product(s) and what to do if items you ve purchased and/or installed are affected. Since the notice includes information for both distributors (like Fromm) and end users (you), we thought it would be helpful to provide highlights of actions items that specifically pertain to you: Inspect the date code on each unit of your inventory which is printed on a small silver sticker located on each unit s carton. If the date code falls between EIA (week 24 of 2012) and EIA (week 3 of 2013), such unit should be returned to Fromm as soon as possible. If installed, you can still inspect the date code: The date code is printed on a small silver or white sticker which is located on the outside of the metal enclosure (see enclosed photo). In the event that you have already installed any units which should have been quarantined and have further questions, please contact Acme toll-free at (press 3 when prompted). Second Product Advisory Notice Transformer Mounting Brackets We would like to thank you for your prompt responses to the Product Advisory dated February 12, 2013 for Acme Electric transformer mounting brackets. Acme Electric has already begun filling the offsetting purchase orders and anticipates having all open orders filled within the next two weeks. Acme Electric will be supplying free replacement brackets for those transformers which fall within the scope of the original Advisory and have already been installed, if such transformers do not appear to be mounted securely. Such replacement brackets have been designed to fit over the existing transformer and can be installed by simply loosening the transformer from its mounted position. In order to notify your customers and/or end-users as to the availability of the replacement brackets, Acme Electric has prepared the enclosed Product Advisory dated February 27, We are asking for your assistance in ensuring that a copy of the new Advisory is sent to each of your customers and/or end-users which have purchased and/or installed a transformer within the scope of the original Advisory. Alternatively, we can send the new Advisory on your behalf if you provide Acme Electric the contact information for your customers and/or end-users. Product Advisory Notice Transformer Mounting Brackets You are receiving this Product Advisory because our records indicate that you may have purchased and/or installed one or more Acme Electric single phase general purpose and buck boost transformers in sizes ranging from 0.25 kva to 2 kva ( Transformers ) at some time during the last eighteen weeks. Since January 25, 2013, Acme Electric has received eight reports that the mounting brackets on some Transformers have come loose either during or shortly after installation. Our investigation has revealed that the mounting bracket failures are directly attributable to a quality issue with the weld between the mounting bracket and the metal enclosure. Please be advised that depending upon how the Transformer is installed or being installed at the time of a failure, such a failure could result in personal injury and/or property damage. A list of the Acme Electric part numbers which may be affected by this quality issue is attached to this Product Advisory. In the interest of safety, and to assure the highest possible quality standards, Acme Electric is asking you to immediately take the following measures: 1. Inspect the date code on each Transformer. The date code is printed on a small silver sticker which is located both on the original shipping box and on the outside of the metal enclosure (see enclosed photo). 2. If the date code falls between EIA (week 24 of 2012) and EIA (week 3 of 2013), and you suspect the Transformer is not mounted securely, has a loose mounting bracket or poses a risk of falling, then please contact the Acme Customer Service Team at to request a free replacement bracket. If the Transformer is securely mounted and/or is not mounted using the welded-on rear mounting bracket, no further action is necessary. 3. If the date code does not fall between the foregoing date code ranges, then the Transformer is not subject to this Product Advisory and no further action is necessary. Product Advisory Notice Transformer Mounting Brackets Acme Electric has been notified of a potential quality issue with the mounting bracket on Acme single phase general purpose and buck boost transformers in sizes ranging from 0.25 kva to 2 kva. This quality condition can result in the mounting bracket coming loose from the transformer box causing the transformer to fall from its mounted position. Depending upon how the transformer is mounted or being mounted at the time of a failure, such a failure could result in potentially serious property damage and/or personal injury. A list of the Acme part numbers which may be affected by this quality issue ( Target Part Numbers ) is attached to this Product Advisory. In the interest of safety, and to assure the highest possible quality standards, Acme Electric is asking you to immediately take the following measures: 1. Issue a stop sale or quality hold on all inventory with any of the Target Part Numbers. 2. Inspect the date code on each unit of inventory which is printed on a small silver sticker located on each unit s carton (see enclosed photo). If the date code falls between EIA (week 24 of 2012) and EIA (week 3 of 2013), such unit should be quarantined and prepared for shipment back to Acme. If the unit has a date code that does not fall within this range, such unit can be returned to stock and made available for distribution. 3. To return quarantined units to Acme, contact the Acme Customer Service Team (CST) and request a Return of Goods Authorization (RGA). An offsetting purchase order for the quarantined units must be placed at the time the RGA is processed. Please your request for an RGA relating to this potential quality issue to: AcmeBrackets@actuant.com. An RMA request form will be ed to you which will include further instructions. Please complete the form, including your purchase order number for the offsetting order in the spaces provided. To expedite processing of your return, please include the following information on the request form: (i) the Target Part Numbers, (ii) the quantities to be returned, and (iii) the original purchase order numbers if available. All returns must be in their original Acme Packaging. If you have any further questions, the Acme CST can be reached toll free at (press 2 when prompted). 4. Concurrent with steps 1-3 above, please contact all customers to whom you have sold any of the Target Part Numbers since June 1, 2012, and provide them a copy of this Product Advisory. If they have inventory within the specified date codes, please arrange to have such units shipped back to you. Please include such customer returns in your original RGA request if possible. Otherwise, please submit additional RGA requests as necessary. In the event that your customer has already installed any units which should have been quarantined, please contact Acme CST toll-free at (press 3 when prompted).
