Søknadsskjema for akvakultur i flytande anlegg Nynorsk Søknad etter lov 17. juni 2005 nr. 79 om akvakultur (akvakulturlova) 1). Søknadsskjemaet er felles for akvakultur-, mattilsyn-, miljø-, vassdrags- og kystforvaltninga. Med unntak av havbeite, som har eige skjema, gjeld skjemaet for alle typar akvakultur i fersk-, brakk- og saltvatn. Ferdig utfylt skjema skal sendast til fylkeskommunen i det fylket som søknaden gjeld for. (Adresse sjå vegleiar). Søkjaren har ansvar for at søknaden inneheld fullstendige opplysingar. Opplysingane vert kravde med heimel i akvakulturlova, matlova, forureiningslova, naturvernlova, friluftslova, vassressurslova, og havne- og farvasslova. Opplysingar som går inn under forvaltningslova 13, er ikkje offentlege, jf. offentleglova 5a. Ufullstendige søknader fører til lengre søknadsprosess, og kan returnerast til søkjaren. Vi viser til ei eiga rettleiing til hjelp ved utfyllinga av skjemaet. Med sikte på å redusere skjemaveldet for verksemdene kan opplysingar som søkjaren har gjeve i dette skjemaet med heimel i lov om Oppgåveregisteret 5 og 6, heilt eller delvis bli nytta også av andre offentlege organ med heimel til å innhente dei same opplysingane.
